How to Qualify

Criteria for Enrollment

  • Certificate of Release or Discharge from Active Duty DD214 or NGB 22 Member 4 Longform. Veterans released or discharged for reasons outlined in 38 USC 5303 and 38 CFR 3.12 d are ineligible.
  • Completed physician form confirming the Veteran or Service Member has a diagnosed service connected PTSI, TBI, or MST, is medically mentally and physically capable of participating in Service Dog training, ownership, required travel, daily exercise, and walking at least one mile, is not currently undergoing inpatient treatment, and is at least six months post successful completion of any substance abuse, mental health, or suicide ideation treatment if applicable.
  • Letter of reasonable accommodation from employer.
  • Must have a stable home environment for at least six consecutive months, for example utility bills or lease, verified by virtual or in person home inspection.
  • Must commit to and participate in all scheduled training sessions.
  • Agreement statement from other adult household members consenting to a dog, if applicable.
  • Individuals convicted of a felony, domestic violence, sexual offense of any kind, or animal mistreatment, abuse, neglect, or cruelty are ineligible.
  • No other dogs reside in the home.

The qualifying criteria are mandatory. You MUST bring all the required paperwork with you at the time of your first interview or we will reschedule the appointment for a later time and date.
